What is an Abscess?

An abscess is a mass that is tender, usually swollen, and can be warm and painful to touch. The middle of an abscess is filled with pus. Any abscess should be treated quickly as infection can spread to other areas of the body and cause some serious consequences. Some of these consequences can be life-threatening.

How an Abscess is Formed

An abscess comes about as the body responds to some infection or introduction of a foreign material. Infections can come from bacteria or a parasite and foreign materials can be injection needles, splinters or bullets. When an infection or foreign object has penetrated the local cells, the inflammatory response of the body is to send many white blood cells to the area to fight the intruder.

In an abscess, a capsule is formed around the area to protect the infection from spreading, and thus the center of the abscess contains pus and the dead organisms that have succumbed in the fight. However, when the capsule is formed, it stops the good cells from reaching and further attacking the causative organisms.

Where an Abscess is Formed

There are a number of different abscesses, named for their location. These include:

•    Bartholin gland abscess – forming around the anus or vagina
•    Axillae abscess – forming in the armpit
•    Pilonidal abscess – an abscess forming around the base of the spine
•    Dental abscess – an abscess that forms around a tooth or in the gum
•    Furuncle – an abscess that forms around a hair follicle (also called a boil)
•    Tubo-ovarian abscess – one that forms in the fallopian tube or in an ovary

The tubo-ovarian abscess is a dangerous one for women as it comes from an infection that has moved up the reproductive tract. The result of such an abscess can be pelvic inflammatory disease, where other structures in the pelvis become inflamed and can lead to infertility. Later stages of pelvic inflammatory disease can mean a life-threatening illness which would take immediate medical help.

Generally, a tubo-ovarian abscess occurs in women who take no precautions against sexually transmitted diseases but who are active sexually. There is also a greater risk of abscesses forming when a woman wears an intrauterine device.

How an Abscess is Treated

Unfortunately, just the use of antibiotics alone cannot commonly cure an abscess. An abscess needs to be opened and drained for it to get well. Some of the time the abscess will drain on its own, but a doctor can also open it with an incision and drain it. For abscesses in the ovaries or fallopian tubes, antibiotics can often stop the infection, but sometimes surgery is needed.
