The Causes of Puncture of the Uterus
Puncture of the uterus is a perforation of the uterine wall. There are a number of medical procedures that can lead to a perforation of the uterus. Someone with a punctured uterus may have abdominal pain, heavy bleeding from the vagina, bloating and other symptoms. Various Causes of Puncture
Usually a perforation of the uterus comes about by the use of a surgical instrument or medical device inserted into the uterus. Some of the causes of a punctured uterus are:
• Insertion of an IUD
• An IUD cutting through the wall of the uterus
• Perforation of the uterus during a dilation and curettage (D&C) procedure
• Forceps used during childbirth
• An instrument puncturing the uterus during induced abortion
• Puncture during a tubal ligation procedure

IUDs and Uterine Perforation
The most common time that uterine perforation occurs with an IUD is during its insertion. The doctor who does the procedure must be well trained in the insertion process or he may insert the IUD too deeply or at the wrong angle. If the uterus is soft, such as after childbirth or an abortion, the risk is greater for perforation to occur. However, an IUD can also be “lost” within the uterus and the end of the device can sometimes perforate the wall of the woman’s uterus.
When perforation has occurred, further complications can arise. The IUD can move into the abdominal cavity and puncture the bowel or bladder as well. It is best to know all the facts about using IUDs for birth control. The Mirena IUD has been known to create this problem for some women.

Those at Risk of Perforation of the Uterus
Some women are more at risk of having uterine perforation. Those who are breastfeeding have a higher risk when they are getting an IUD inserted or if they are getting a D&C. These women need to be carefully monitored during and after the procedures. Postmenopausal women also have a higher risk because as they age the cervix becomes narrower and the walls of the uterus become thinner.
Women who have abortions in the past also have a higher risk of perforation of the uterus. In fact, about one out of every 250 abortions results in uterine puncture. This rate is higher when the abortion is done in the second trimester.
Women who have had C-sections or surgery on the cervix have a higher incidence of puncture of the uterus. This is due to the fact that scar tissue can form during these procedures. This scar tissue can weaken the tissues of the cervix, leading to an increased risk of perforation.
