How Does a Doctor Insert an IUD?
It’s important to understand what will happen when you walk into your doctor’s office to get an IUD so you can take the initial steps to prepare for the insertion of your IUD.  Make sure that your doctor has explained to you in detail exactly what will occur so that you will be more at ease, preventing the procedure from being painful or uncomfortable.  This will help the insertion process go quickly and easily.

The Examination
The first steps to having an IUD inserted by your doctor will be to have bimanual examination where the doctor will place two fingers in the vagina and the other hand on your abdomen.  This examination can help identify the position, size and consistency of the uterus and check for any signs of infection or tenderness.


Preparation
Once your doctor has determined there are no obvious complications, he or she will hold the vagina open using a speculum, a medical tool that looks similar to a duck beak.  The doctor may then use a local anesthetic, which should be discussed prior to the procedure, to reduce discomfort.  Your doctor will then use a long-handled instrument, called a tenaculum, which will be attached to your cervix to stabilize the uterus.

Once your doctor has set the two above-mentioned instruments inside the vagina, he or she will insert an instrument that is called a sound to measure the cervical canal in both length and direction.  This can reduce a chance of a uterine puncture because it will prevent the IUD from being inserted in the wrong angle or too deeply.  Once this has been done, the doctor will remove the sound and prepare the IUD to be inserted.


Insertion
The arms of IUD are swept back and inserted into a tube.  It is then pushed into place at the correct depth using a plunger-type device.  It is completely normal to feel a bit of discomfort in the form of cramping or pinching during the procedure.  The IUD will be positioned correctly by your doctor as quickly as possible to avoid any excessive discomfort.

Post Insertion
At this point the doctor will then remove the plunger tube, followed by the tenaculum and then the speculum.  Your doctor may cut the strings attached to the IUD so that they are only and an inch or two into the vagina from the cervix.  These strings won’t be seen, but can be felt by inserting a finger into the vagina. The doctor will instruct you as to how to check to see if your IUD is still in place by finding the strings.
As your doctor will no doubt inform you, you may have spotting or slight bleeding for a few days after the insertion of the IUD.  There is nothing to worry about as this is normal and should be expected. You may also experience a heavy first period that may come a day or two earlier.  Ask your doctor if you have any questions or if you experience severe pain or discomfort.
