Types and Placement of IUDs

IUDs are the most widely used forms of contraception in the world. There are an estimated 160 million people worldwide who have these devices implanted. The “T”-shaped device is inserted in the uterus and releases either copper or progesterone to prevent pregnancy. Many women prefer this method of reversible contraception as it is long-lasting with no attention needed other than getting regular checkups.

Types of Intrauterine Devices

Two types of IUDs are available in the United States. These are a hormonal IUD manufactured by Bayer under the name of Mirena and a non-hormonal IUD made of copper called ParaGard. The hormonal IUD remains implanted for five to seven years and the copper IUD can last for ten years.

Hormonal IUDs are devices that release small amounts of hormones into the system. The amount put into the system is less than other birth control methods using hormonal treatment. The devices are known to reduce menstrual bleeding, leading to the use of these IUDs for women who have heavy periods or menorrhagia. The hormones themselves only affect the uterine area where with other contraceptives that release hormones, the progesterone reaches other parts of the system.

Copper IUDs disrupt sperm through the use of copper. The copper is a natural spermicide that acts within the uterus. The “T” shape has copper wire wound around the frame, with the arms of the “T” holding it within the uterus. Some of these devices are made with a gold core which prevents corrosion or fragmenting. These IUDs can be used as contraception in an emergency. They can prevent conception for as long as five days after unprotected sex. Disadvantages include increased menstrual bleeding and cramps.

Placement of IUDs

A physician in the United States worked out the design of the intrauterine device based on the shape of the uterus. When the uterus contracts, it forms a “T” and the doctor determined that the rate of expulsion of the device would be far less if this was the shape of an IUD.

Prior to a woman having one inserted into the uterus, her doctor must do a thorough physical exam. He must ensure that all her reproductive organs are normal and that she has no infections. He will ask about her medical history and query her about her lifestyle.

The IUD is inserted during an office visit with the doctor. The best time to have it done is immediately after a woman’s menstrual period when the cervix is soft and pregnancy is unlikely. The doctor will use an instrument called a speculum which holds open the vagina. A tube is used to place the device. Once inside the uterus, the arms of the “T” open and hold it in place.

Effects Once Inserted

After an IUD is inserted, a woman may have some cramping and some may feel dizzy. The patient may be instructed to get and take an over-the-counter pain reliever, which if started before insertion may prevent cramping. Some doctors may prescribe antibiotics to prevent infection.

After placement of an intrauterine device, a woman can continue with her normal activities. She can exercise, go swimming and have intercourse. She may also use tampons at any time after an IUD is inserted.
