Diagnosing Perforation of the Uterus
Diagnosing perforation of the uterus is not always simple because at times there are no real symptoms that show up. The perforation of the uterus can mean serious complications, however, so if there is any sign or symptom, a woman should get an exam to find if perforation occurred. If the woman wears an IUD, has had a D&C procedure, or had an induced abortion, she could be at risk of uterine puncture.

Symptoms Indicating Perforation of the Uterus
A woman should be alerted to the potential of having her uterus perforated if the string to her IUD disappears. This could mean that the device moved and could have punctured the uterine wall. For uterine puncture from any cause, the woman may feel pain in the lower abdomen, have bleeding from the vagina, feel bloated or have nausea or vomiting. She could also run a fever and experience rapid heartbeat.

Other signs and symptoms the doctor would look for in diagnosing uterine puncture would be the following:
• Cramping
• Irregular menstrual cycles
• Skin changes and problems
• Back pain
• Weight gain
• Hair loss
In addition to the above symptoms, a woman may notice a decreased sexual drive and mood swings. More serious symptoms could indicate that an IUD continued to move from the uterus and punctured other pelvic organs, such as the bladder or intestines.
The Physical Examination by the Doctor
When a doctor does a physical exam he will be examining the uterus, looking for signs that the uterine wall has become soft due to past childbirth or from an abortion. He will do a palpitation of the abdomen to note any abdominal distention. For women who are postmenopausal, he will see if the cervix has narrowed and if the walls of the uterus have become thinner with age. This will help him determine if uterine puncture is likely.
After taking any medical history, he will know to look for possible perforation if the woman has had a C-section before or had surgery on the cervix. He will watch for any scar tissue that was formed that could lead to easier uterine perforation.
