The intrauterine device is among the most effective birth control methods, with an annual failure rate of less than 1%. But the IUD isn’t just one product; there are five different IUDs available on the US market. Deciding which is right for you is easier if you have a clear understanding of how they differ.
Paragard
While IUDs have been used throughout the 20th century, the idea of adding copper to the devices was first conceived in the late 60s, and the Paragard was invented in the 1970s. it’s a plastic T-shaped device with copper coiled around it. It works by making the uterus a hostile environment that kills sperm. The ParaGard is the only IUD on the market to be completely nonhormonal, making it a top choice for women who want long-term contraception but fear hormones may cause side effects. It remains effective for 10 to 12 years. Its top side effects are an increase in menstrual bleeding and cramping.

Mirena
Hormonal IUDs were developed in the 1970s; researchers realized that while the presence of a foreign body in the uterus alone can prevent pregnancy, adding hormone could increase the effectiveness. Today the Mirena, a plastic t-shaped device, is the best-known hormonal IUD. The Mirena releases approximately 20 mcg/day of progesterone into the uterine cavity, changing the composition of the uterus and in some cases preventing ovulation. The progesterone mainly stays in the uterus and has little effect on weight or mood compared to methods that release hormones into the bloodstream, like the pill or patch.

The Mirena remains effective for five to seven years. It is among the most effective birth control methods. One side effect many users consider a plus is its tendency to reduce or even stop uterine bleeding.
Kyleena IUD
The Kyleena is made by the manufacturers of Mirena, Bayer Pharmaceuticals, and has many similarities. Its main difference is that it’s slightly smaller than the Mirena and releases slightly less hormone (17.5 mcg a day compared to 20). It has been marketed as a good choice for women who have never had children (due to the smaller size of their uterus and slightly greater difficult involved in dilating their cervix to insert objects). But the Mirena is also a perfectly suitable choice for childless patients. Both devices have high effectiveness, the same length of use, and the same side effects (including reduced periods).

Liletta
The Liletta is not drastically different from the Mirena: it has basically the same design, drug does and effectiveness. Its one dramatic difference is in price. A site for healthcare providers reports that “Cost is the only place where there may be a profound difference between the two devices. Liletta is the result of a unique partnership with women’s health advocates, researchers, and a not-for-profit drug company to develop an affordable IUD. Throughout the process, Liletta’s makers have been committed to ensuring the device is broadly accessible, even to those without health insurance and those with high deductibles. Their goal is that no one will ever have to pay more than $150 for a Liletta.” Uninsured patients seeking IUDs should seek the Liletta out.
Skyla
Skyla is like Kyleena in being marketed to younger, childless women (they’re the same size, slightly smaller than the Mirena). It’s the lowest in hormone does of the four, releasing 14 mcg a day compared to Mirena’s 17. The Mirena may be better for those who want the highest level of period relief, while Skyla is the choice of those who like the idea of less artificial hormone.
All five options are highly effective and appropriate whether or not you’ve had children. They also share the same serious side effects—a rare risk of uterine perforation or pelvic infection, and resulting damage to tissue. Patients should consider all the pluses and minuses to make their choice.
