Most women who use an IUD for birth control won’t experience serious side effects. But those that do occur can be painful or dangerous, or put them at risk of unwanted pregnancy. One such problem is expulsion, in which the small device becomes dislodged and travels out the cervix. While relatively rare, the risk of this is increased significantly by use of a menstrual cup.

Menstrual cups are plastic or rubber cups that are worn up near the cervix to collect menstrual blood. They’re touted as environmentally friendly and free from the substances in tampons, such as bleach used to whiten cotton, that some fear may cause harm. Since they can be emptied and reused, they also free users from the problem of running out of tampons or pads.

But as recent research shows, there’s a problem. A 2020 study published in Obstetrics & Gynecology followed 1,092 women with copper IUDs. Seventeen percent of women who used menstrual cups had their IUD dislodge, compared to five percent of those who did not—a more than threefold difference.

Most likely causing this result is that the menstrual cup creates suction that may pull on the IUD when it’s being removed. Creating a seal within the upper part of the vagina helps the cup stay in place and prevents spilling. When a user is ready to remove the cup, they are supposed to press on one side to break the seal first. But that step is either not being followed, or is not enough to prevent suction from developing.

It’s also possible that IUD strings are catching on the cup and being pulled out with it. Strings are left protruding from the cervix to make eventual removal of the IUD easy; the position they settle in and the length they’re trimmed to could determine the chance they will interfere with a menstrual cup.

In one third of displacement incidents in cup users, the IUD came out during cup removal. The high rate of displacement suggest that even in cases where it fell out at another time, the cup had an effect on this outcome.

The results were so significant that the researchers began advising the women not to use a menstrual cup after the first year, but this had no effect on rate of use.

For now, no official guideline exists on the use of menstrual cups with IUDs; in the future, research like this could prompt health caregivers to warn physicians about the risks of this combination.

Both copper IUDs and menstrual cups sometimes inspire loyalty and enthusiasm among those who use them. But for best results, it may be wise to choose one or the other, not both.
